SUPREME SHATIN

Personal Information Collection Statement

Version PICS-v1.2. Applies to membership applications and member services of Supreme Shatin Football Club.

This English text is a translation provided for convenience. The Chinese version is the official statement; if the two differ, the Chinese version prevails.

The information you write on the membership application form is collected and used by Supreme Shatin Football Club (the “club”). This statement sets out what the club collects, what it is used for, who it may be given to, how long it is kept, and how you can ask to see or correct it. Giving the information is voluntary, but without the items marked as required below the club cannot process your membership application.

1 · What the club collects

2 · What it is used for

3 · Direct marketing needs your separate consent

The club will not use your name, phone number or email to send you promotional messages about events, ticket sales, renewals or sponsors without your consent. There is a separate box on the form for you to choose whether to receive them; leaving it unticked has no effect at all on your application or your membership. You may withdraw your consent at any time by contacting club staff, and the club must then stop using your information for promotional purposes.

Displaying your photo or portrait (for example on the Members’ Wall at the ground) is a separate consent again, counted apart from promotional messages. Only founding members are asked for it, because the Members’ Wall is a founding-member benefit.

4 · Who your information may be given to

The club transfers your information only as far as the purposes above require. The recipients may include:

Unless you have separately consented to direct marketing, the club will not transfer your information to any third party for that party’s own promotional purposes, and will not sell your information.

5 · How long it is kept

The first 4 characters of your ID card number are used to approve your application. Once it is approved they are kept on your member record and used during your membership to confirm who you are (for example when you contact the club, change your details or collect a member benefit). The copy on the application itself is deleted when the application is approved; if an application is rejected, it is deleted 90 days after the rejection — those 90 days exist so the club can answer whether this applicant has applied and been rejected before. The ID card number on your member record, and the rest of your information, are kept for the duration of your membership and for as long afterwards as is reasonably necessary to handle accounts, disputes and legal obligations, and are destroyed once they are no longer needed.

6 · Access and correction

Under the Personal Data (Privacy) Ordinance you have the right to ask for access to, and correction of, the personal data the club holds about you, and to ask the club to stop using your data for direct marketing. Please email jim@supremeshatin.com to do so. The club may charge a reasonable fee for an access request.

7 · The version of this statement

The club records which version of this statement you agreed to. A later revision does not automatically widen a consent you have already given; a new purpose requires the club to ask you again.
